Experience working with underserved populations with medical and social needs in a social service and/or community-based setting is preferred Education • Associate's Degree in social work, health/human services, community/public health or related field and 3 yrs related experience (social work, health education, community outreach, community health or human services) OR • Bachelor's Degree in social work, health/human services, community/public health or related field and 1 yr related experience (social work, health education, community outreach, community health or human services) • Bachelor's in Social Work/BSW prepared is preferred Specialized Knowledge and Skills • Knowledge and use of community resources; social, health and public agencies and services. General Requirements • Obtain Basic Life Support (BLS) certification issued by the American Heart Association within 45 days of hire and maintain • Community Health Worker certification is preferred Work Experience • See education for required experience.
