Description Summary:The Emergency Department Navigator helps the patient and/or caregiver navigate the complex healthcare system by complimenting the services delivered through the Emergency Department. The Emergency Department Navigator does not provide clinical care and does not extend or substitute for the more specialized services of a doctor, nurse, or social worker.Responsibilities:<ul><li>Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.</li><li>Bridge cultural barriers between communities and the healthcare system</li><li>Provide culturally appropriate and accessible health education and information</li><li>Assure that people get the services they need</li><li>Provide informal counseling and social support</li><li>Advocate for individuals and communities within the health and social service system</li><li>Identify barriers and circumstances that led the patient to the emergency department</li><li>Navigate the complex healthcare system by:</li><li>Providing patient with information and linkage to a primary care provider if patient does not have a provider</li><li>Scheduling follow-up appointment with provider that patient will keep</li><li>Providing patients and family caregivers with information and linkages to health and social support services including discounted prescription medications and transportation</li><li>Providing patients with complex needs with direct referrals to the Community-based Community Health Workers for continued follow-up and assistance in the community</li><li>Contacting patient within 48 hours of discharge as follow-up</li><li>Understand about medical homes and the proper use of primary care and the Emergency Department.</li><li>Develop Navigation and Accountability Plan</li><li>Excellent customer friendliness and communication</li><li>Know available Community Resources and how to refer appropriately to identify needed services.</li><li>Perform all other duties as assigned.</li></ul>Requirements:Education/Skills<ul><li>High School Diploma or equivalent required</li><li>Associate or Bachelor’s Degree preferred</li><li>Bilingual (Spanish) preferred</li></ul>Experience<ul><li>Healthcare background preferred.</li></ul>Licenses, Registrations, or Certifications<ul><li>BLS preferred</li></ul>Work Schedule: PRNWork Type: Per Diem As Needed
