Other services include the Health Related Social Needs (HRSN) program, which works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection. The Enrollment and Care Coordination Specialist will screen indivdiuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member.
