Collaborates with members of the community including schools, medical providers, psychiatric providers, DCP&P staff, treatment facilities and other community resources that the youth and family are engaged with to support assessment, treatment, service planning, and transition. We have opportunities to serve families in the following New Jersey counties: Ocean, Burlington, Camden, Passaic, Bergen, Hudson, Somerset, Hunterdon, Warren, Mercer, Monmouth, Morris, Sussex, Union, Essex, and Middlesex.
