Assists the Director of Admissions and Family Services with Transition Planning for assigned student, as appropriate, whether to adult services or to a less restrictive environment (e.g., prepare Summary of Student Performance, attend transition planning meetings or assist in compiling needed paperwork (re-evaluations, psychological reports, IEPs, etc.) Facilitates timely completion of PASA/alternate assessments and collaborates with the supervising BA throughout this process.
