Coordinate with the agencys Transition Programs staff, including Clinical Care Coordinators (CCCs) and Transition Specialist, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists, Partners with the local HUD Continuum of Care, public housing authority(ies), and affordable housing owners and property managers. Responsibilities for the Mental Health Professional I/Housing Coordinator: Provide oversight for the Centers housing programs, including Community Housing, Homeshare, Homeshare Respite, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCFs), and other supportive housing programs, as applicable.
