Supported living assessments• Carry out specific skills training plans for participants as instructed• Assists participants in all personal care needs (toilet needs, feeding and hygiene)• Assist participants with their mobility and communication needs• Develop natural and community support• Assist in medical appointments and health needs• Maintain detailed documentation of services provided• Assist with preparation of reports on participants• Participate in regular staff meetings• Maintain appropriate communication with supervisor(s)• Other duties as assignedPHYSICAL DEMANDS:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
