D. For time sensitive patient complaints - immediately contacts charge nurse or designee for patients in distress, shortness of breath/difficulty breathing, chest pain, altered level of consciousness, complaints of new onset numbness or new onset difficulty speaking, uncontrolled bleeding and severe pain. A report communication form will be completed on each patient to include: patient's current vital signs, current heart rhythm, basic demographic data, interventions, summary of patient complaint and estimated time of arrival.
