Utilizing clinical judgment, evidence-based practice, and a person-centered approach, the D-SNP Care Manager RN conducts comprehensive assessments, develops and implements individualized care plans, coordinates interdisciplinary care, and supports members across the continuum of care. Coordinate with Medi-Cal Community Supports providers, transportation vendors, and internal transportation teams to arrange and monitor services that address members' medical, functional, and social needs, supporting adherence to care plans and reducing barriers to care.
