Collaborate closely with a cross-functional care team - including PCPs, specialists, and other the company team members such as pharmacy, clinical nursing, and social work - as well as interfacing with family members and caregivers as needed to coordinate holistic care for the member, to ensure continuity of care and deliver a collaborative care plan. Leverage Digital (live portal chat) and AI-assisted clinical tools - including agent-supported prescribing and inbox triage workflows - to improve efficiency, consistency, and safety in medication renewal, results management, and patient communication, while providing clinician feedback to help refine these tools as they evolve.
