Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout Brown County, Ohio and surrounding Southern Ohio areas in a Part-Time capacity.
