You will collaborate with hospitals, physician offices, long-term care facilities, emergency departments, and internal clinical teams to ensure patients are transferred at the right time, to the right place, and at the appropriate level of care. In this critical role, you will serve as the centralized point of contact for all acute EMS patient transfers into, and across, our organization – ensuring safe, timely, and seamless transitions of care for patients and providers across the region.
