Your duties include the preparation and dispensing of drugs in all inpatient settings; review and verification of medication orders and patient medical records; optimizing safe medication therapy; review of patient medication profiles; counseling patients on appropriate drug use; interacting and/or rounding with other health professionals to help provide safe and effective use of medications; provision of advanced clinical services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN and aminoglycoside monitoring, pharmacist interventions, anticoagulation) to neonatal, pediatric and adult patients according to departmental policy and procedures, with possible focus on pharmacy specialty (i.e., oncology, pediatrics, geriatrics, ambulatory care); oversee dispensing, return and waste of controlled substances on assigned shift/unit. As a condition of employment, the final candidate who accepts an offer of employment will be required to disclose if they have been subject to any final administrative or judicial decisions within the last seven years determining that they committed any misconduct; or have filed an appeal of a finding of substantiated misconduct with a previous employer.
