The Social Worker collaborates with physicians, advanced practice providers, nurses, therapists, case managers, and other members of the interdisciplinary care team to address patients' emotional, behavioral, social, and practical needs. Facilitate referrals to behavioral health providers, community agencies, support groups, financial assistance programs, housing resources, transportation services, substance use treatment, and other appropriate resources.
