This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. As a Branch On-call Registered Nurse you will:<ul><li>Assume primary responsibility for the patient/family caseload that includes assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team</li><li>Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift</li><li>Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information</li><li>Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays</li><li>As scheduled, attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient/family needs</li><li>Under the physician’s order, admits patients eligible for hospice services in collaboration with the triage team</li><li>Reports patient status and need for other disciplines to clinical leadership, attending physician and hospice physician</li><li>Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and/or holiday(s)</li><li>Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care</li></ul> Qualifications Required Knowledge, Skills and Experience<ul><li>Is currently licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)</li><li>Current driver’s license and automobile in good working condition with proof of auto insurance </li><li>Two years recent experience as a professional nurse</li><li>Is able to meet certain physical demands of standing, bending, lifting, stooping or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16 to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time)</li><li>Ability to work in a field setting and exhibited ability to make sound nursing judgments; must be able to work independently</li><li>Ability to assess patient needs and formulate individualized patient care plans to meet those needs</li><li>Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers</li><li>Effective written and verbal communication skills</li></ul>  Preferred Knowledge, Skills and Experience<ul><li>Two years’ experience within hospice</li><li>Certified Hospice and Palliative Nursing</li><li>Electronic Medical Records Experience</li></ul> Pay Transparency  Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
