You will achieve this goal by building relationships with patients and physicians, coordinating the defined plan of care, assisting with appointments, assisting in resolving barriers, providing education, and connecting with community resources. Supports providers by assisting patients and/or families in understanding their diagnosis, treatment options, and the resources available, including education, clinical research studies (as applicable), and technological advances.
