Case management may include linking with providers and maintaining list of relevant referral sources, supporting treatment planning efforts, coordinating care needs, navigating on campus barriers and submission of documents (financial aid, emergency withdrawal, post-hospitalization success planning, accommodations with disability), conducting follow-up to ensure referral connection was made/care received and assist in removing barriers to care. Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services; assists client to develop natural resources and make contact with social support networks .
