Build lasting relationships with referral sources, including hospital case managers, rehab and skilled nursing discharge planners, physicians’ offices, Area Agency on Aging and ADRC staff, and managed care organization (MCO) care managers. You will build and maintain referral relationships with hospitals, discharge planners, social workers, care managers, senior living communities, elder law attorneys, and community organizations.
