West Territory: Bellwood, Forest Park, Maywood, Westchester, Broadview, Stone Park, Berwyn, Chicago Ridge, Joliet, Lemont, Oak Lawn, Bridgeview, Hometown, Justice, Burbank, Palos Park, Palos Hills, Summit, Downers Grove, Burr Ridge, Lyons, and west/southwest Chicago. The Registered Nurse will assess patient needs, develop and coordinate individualized plans of care, perform skilled nursing procedures, supervise assigned clinical personnel, and help patients safely manage their health at home.
