As a member of the Interdisciplinary Team, this role completes home-based assessments, supports care planning, monitors service delivery, coordinates with contracted providers and community-based resources, and provides nursing care within the Licensed Vocational Nurse scope of practice as needed. Monitor participant condition and home care service delivery through collaboration with home care staff and external agencies, and report changes in condition, treatment concerns, or adverse reactions to the Registered Nurse, Physician, or other appropriate team members.
