p>The Clinician (Care Coordinator Receiving Center) is responsible for screening and assessing all youth admitted to the center, providing therapeutic complex trauma informed interventions to youth, work on the floor with youth, developing safety and service plan, train staff on plans, model and train child care worker staff on effective ways to engage and stabilize the youth, liaison with county social worker, nurse and other youth serving entities.

Assist with daily living tasks: grooming, bathing, toileting, and personal hygiene Support meal prep, light housekeeping, errands, and laundry Provide safe mobility support and help with transfers Accompany clients to appointments and help them live more independently.

Serving as a key member of the patient access and care coordination team, you will help facilitate timely admissions, support exceptional patient experiences, and ensure referral partners receive responsive, high-quality service. This position is responsible for managing referral intake activities, verifying patient information, coordinating with internal teams, and maintaining strong relationships with referral partners to support timely and efficient patient admissions.

li>Participate in regular team meetings, training sessions, and workshops to stay informed about the latest developments in social services and care management. While the incumbent will have flexibility to work from home, they are expected to travel extensively—approximately 50% to 80% of the time—within Santa Cruz County.
