What You'll Do:• Coordinate and manage patient care services• Serve as a liaison between patients, families, providers, and caregivers• Manage referrals, authorizations, scheduling, and care plan coordination• Participate in interdisciplinary case conferences• Monitor patient progress and ensure continuity of care• Support quality assurance, chart audits, and regulatory compliance• Collaborate with clinical and administrative teams to improve patient satisfaction and outcomes. Qualifications: Active California PT, OT, or SLP License Current BLS Certification Home Health experience preferred Knowledge of Medicare regulations and OASIS preferred Strong organizational, communication, and computer skills Leadership, care coordination, or management experience is a plus.
