div>Key Responsibilities
Conduct client assessments, reassessments, supervisory visits, care plan reviews, and progress evaluations.
Review and update individualized care plans based on client needs, condition changes, family feedback, and care team communication.
Complete caregiver supervisory visits to observe care delivery, ensure care plan compliance, and provide coaching or guidance as needed.
Provide clinical support, training, and direction to caregiving staff to promote safe, high-quality care.
Communicate professionally with clients, families, caregivers, physicians, office staff, and the Director of Nursing.
Maintain accurate and timely clinical documentation in WellSky.
Support quality assurance, chart reviews, documentation accuracy, and compliance readiness.
Identify changes in condition, safety concerns, or care plan needs and communicate appropriately.
Conduct medication box pours and ensure medication management procedures are followed.
Assist with clinical problem-solving, client concerns, caregiver support, and ongoing care coordination. Private-duty home care, home health, hospice, assisted living, or similar experience preferred.
Experience with assessments, reassessments, supervisory visits, or field visits preferred.
Strong clinical judgment, communication, organization, and problem-solving skills.
Ability to work independently while collaborating with the office and nursing team.
Comfortable using electronic documentation systems; WellSky experience preferred.
Valid driver’s license, auto insurance, and reliable transportation required.
CPR certification, TB screening, and Level 2 AHCA background screening required, or willingness to complete.
