p>Preferred Qualifications: • Experience in Value-Based Care or Population Health, including Medicare Advantage documentation and Health Risk Assessments • 5+ years' experience as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology • Experience managing population health with a focus on reducing hospitalizations • Prior experience with Cerner or similar EHR systems.
Clinical Care & Evaluation:
The primary responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions while addressing medical, social, emotional, and financial needs.
Education and Experience Required: Bachelor's degree in Accounting, Finance, Economics or related field, and a minimum of 5 years' risk management experience or in lieu of degree, a combined minimum of 9 years' higher education and/or work experience including a minimum of 5 years' risk management experience. As such, the position interacts with middle and senior Treasury management, as well as management within the business lines to obtain balance sheet forecasts and an understanding of both strategic and tactical initiatives that may impact the institution's risk profile.
