Demonstrates excellent oral and written communication skills to: document care given, delegate assignments, relay information, make referrals, and act as liaison between patients, physicians, other hospital disciplines, and community agencies. Assesses patient needs, identifying problems, planning and implementing interventions, delegating activities appropriately, evaluating the effectiveness of the care given, and adjusting the plan accordingly, using the nursing process.
