You will coordinate housing and transportation resources, build partnerships with landlords, housing authorities, transportation providers, and other stakeholders, monitor contract requirements and project deliverables, and work with internal teams to address barriers, improve access, and support successful transitions. What You'll Do: Manages housing and transportation initiatives in alignment with contract requirements and transition goals, coordinating day-to-day activities, maintaining workflows, and escalating urgent or recurring barriers that could affect member safety, transition readiness, service access, or continued community living.
