Preferred: Bachelor's Degree in a related field such as Social Work, Psychology, Education, Counseling, Criminology or Social Welfare, Child Development, or other relevant fields especially with regards to TAY homelessness and vulnerable populations and/or social services. Case Management is a collaborative process of assessment, planning, facilitation, care, coordination, evaluation, and advocacy for options and services to meet an individual’s (and Family) comprehensive needs through communication and available resources to promote safety, quality of care, and cost-effective outcomes.
