SPECIAL SERVICE FOR GROUPS, INCJOB DESCRIPTION Title : On-Site Case Manager Division : HOPICS/PATHWAY HOME FLSA : Non-Exempt, Full time Supervisor : Program Manager Pay Range or Rate : $23.00 per hr. to $32.79 per hr Summary Under the direction of the Pathway Home Program Supervisor and Program Manager, the On-Site Case Manageris responsible for screening, on-site case management, eligibility, service coordination, and triage for InterimHousing placement and transition care to permanent housing case management Pathway Home participants. Minimum Qualifications - Knowledge, Skills and Abilities RequiredAssociate degree in social services or other administrative, business or housing field is required from anaccredited or state approved college or university, with a minimum two (1) year job related experience workingwith homeless individuals and families OR 1 to 2 years’ experience working in social service field; casemanagement and homeless program experience preferred.
