Summary: In concert with the Director, Care Management and/or Manager, Care Management, this position is responsible to supervise the day-to-day operations of the Care Management Department to ensure consistent, effective and efficient site-specific medical management, discharge planning and care coordination for patients across our continuum. Serves as subject matter expert and coach in terms of effective and timely discharge planning, care coordination including identification and resolution of patient and family concerns, insurance issues and/or discharge to other appropriate levels of care.
