Job Summary: Under supervision of a Licensed Behavioral Health Case Manger (Licensed Clinical Social Worker), provides integrated and coordinated case management services for the complex, high-risk and high-utilizer populations in variable psychiatry, mental health, and addiction medicine settings. Consistently supports compliance and the Code of Ethical Conduct by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state, and local laws and regulations, accreditation, and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures.
