The ideal candidate will work closely with a dedicated interdisciplinary team-including an Advanced Practice Provider (APP), nurse care manager, behavioral health therapist, and clinical pharmacist-to deliver high-touch, coordinated care to a defined patient panel. In this role, you will collaborate with other physicians, nurses, therapists, and pharmacists to deliver comprehensive care to patients enrolled in value-based care programs, such as Medicare Shared Savings Programs (MSSP).
