PHYSICAL REQUIREMENTS: • Able to speak clearly and distinctly with staff, physicians, patients, and families • Able to extensively utilize telephone for communication • Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule • Able to sit/stand/walk for extended periods of time throughout the work schedule, with some light lifting and bending • Demonstrates adequate vision, or correctable with glasses/contacts, for reading and preparing written documents • Must have adequate perception of sounds or adequate hearing with corrections • Able to hear spoken communications from staff, physicians, patients, and families as well as pages • Demonstrates adequate manual dexterity to prepare written documents • Able to work in a latex burdened environment. JOB SUMMARY: The case manager is a RN or LPN who collaborates with interdisciplinary team members, patients, and families to facilitate the transition of the patient from the hospital to a post-hospital setting.
