Serves as the pharmacy clinical specialist for the benign hematology practice and works collaboratively with hematology providers and the interdisciplinary care team to optimize medication therapy, improve patient outcomes, and support safe, evidence-based, and cost-effective care. In conjunction with a patient''s healthcare provider, provides, delegates, and coordinates patient care, which may include medication initiation and adjustment, laboratory monitoring, point-of-care testing, and follow-up care within applicable scope of practice and collaborative practice agreements.
