Triages, assesses, diagnoses, determines the most appropriate level of care, and coordinates appropriate referrals for behavioral health patients requiring crisis assessments in the Emergency Department and inpatient units, Coordinates and/or completes all assessments and goals that are strength-based, solution-oriented in order to build treatment plans from admission to discharge, with family and service providers. The Behavioral Health Specialist provides behavioral health consults, assessments, individual treatment, family consultations, care coordination, referrals, and participates in multidisciplinary teams which coordinates, monitors, and plans behavioral health care services throughout the episode of illness to meet the individual needs of our patients in order to improve the quality of care.
