Deliver role functions in the program/site, client’s home, community, collateral contact's office, DCF, DMH, DPH, DDS, or school building, or other setting that is appropriate for the client in a safe and appropriate manner. ALP helps individuals obtain a housing subsidy, secure an apartment and then provides ongoing case management to help each individual maintain their health, housing and independence.
