Obtain Medication Histories and allergy information for new admissions to the hospital in a timely manner, utilizing all possible resources (i.e., communication with patient, medication history lists, medications brought in by patient, physicians (family or current attending), nursing, retail pharmacies, or other medical facilities where the patient may be transferring from. The Pharmacy Medication History Technician must communicate effectively and courteously with patients and health care providers, using interview skills, listening techniques, medication knowledge, and available resources to develop a complete and accurate electronic list of medications and allergies for each patient.
