Luke's University Health Network (SLUHN) Home Health and Hospice to build and enhance relationships with referral sources throughout the Network and Community; develops and plans educational programs, visits provider offices, nursing facilities, assisted and personal care communities to promote the Home Health and Hospice philosophy of SLUHN; participates in Community events to promote and educate the public at large about the SLUHN Home Health and Hospice services; and works closely with the Director of Clinical Referrals and Liaisons, Hospice Liaisons, Development and Marketing departments to coordinate these outreach efforts.
