The LPN Home Care Coordinator completes participant visits, provides nursing support within the LPN scope of practice, communicates changes in participant condition, and helps ensure care plans and services are carried out effectively for our PACE participants. Provide direct participant care in the home setting, including but not limited to wound care, catheter changes and flushing, medication reconciliation, acute symptom management and triage, vital sign monitoring, and laboratory specimen collection, etc.
