Monitoring and maintaining provider profiles, panel management including capacity, patient/ panel load leveling, suggestions with supporting data to the Primary Care Service Chief, oversight and correction of transmission efforts, and review/recertification of incorrect patient assignments, ensuring timeliness and accuracy of data input, monitoring tracking and management of any Primary Care Electronic Wait Lists (EWL) and identification and appropriate disposition of patient with duplicate PCMM assignments elsewhere within the VISN and VHA. Electronically create or modify team and position settings, assign provider user class based on verified credentials, set maximum panel sizes (based on analysis of provider licensure, FTE, patient intensity score, support staff, treatment rooms and specialty PACT adjustment factors), program clinical alert notification levels and establish preceptor to associate provider links and ensure proper reporting structure to national VSSC database.
