Work Experience: Experience with program development, education programming, or community-based health initiatives, collaborating across interdisciplinary teams, including clinical, operational, and community-based partners, supporting or participating in community outreach, health education, or public health initiatives, and providing leadership, coordination, and oversight of programs, initiatives, or interdisciplinary teams. Acts as an Epic Care liaison for maternal health workflows, coordinating with Registration and HIM to create pregnancy episodes for patients receiving care outside of Epic-enabled practices and ensuring accurate input of critical prenatal data, including laboratory results, to support visibility of expected deliveries and continuity of care.
