This position works collaboratively with patients, healthcare providers, care teams, community agencies, and other stakeholders to identify patient needs, develop individualized service plans, coordinate referrals, monitor progress, and promote improved health outcomes. The Case Manager serves as an advocate and resource for patients while helping reduce barriers to care and connecting individuals and families with appropriate medical, behavioral health, social service, financial, housing, transportation, and other community resources.
