Assist residents with Activities of Daily Living (ADLs) including, but not limited to continence care, showering, grooming, dressing, escorts, transfer assistance, repositioning, and meal assistance. Observe, document, and report to the Care Manager Team any physical, emotional, or mental changes, as well as unusual resident behaviors (e.g., speech, feeding patterns, toileting), to support assessment and care planning.
