POSITION RESPONSIBILTIESOversee all daily operations of the Medical Respite Program, including admissions, discharges, census management, and service delivery coordinationEnsure appropriate program flow and bed utilization by coordinating referrals, admissions decisions, and discharge timelines in collaboration with referral sources and internal staffDevelop and implement operational procedures to support efficient, consistent, and high-quality service deliveryProvide direct supervision, guidance, and ongoing support to program staffSupport staff in managing complex client situations, including crisis intervention, conflict resolution, and client advocacyFacilitate regular team meetings, case conferences, and supervision sessions to promote communication, accountability, and best practicesIdentify training needs and support staff development, particularly in areas such as trauma-informed care, harm reduction, and medical respite best practicesCollect, analyze, and report program data related to client outcomes, service utilization, and operational performanceMonitor key performance indicators such as length of stay, hospital readmission rates, successful discharges to housing, and client satisfactionUse data to inform program improvements, identify trends, and support strategic decision-makingBuild and maintain strong relationships with hospitals, healthcare systems, shelters, and community-based organizations to support referrals and continuity of careServe as the primary point of contact for external partners regarding program operations, admissions, and coordination of careRepresent the program in community meetings, provider networks, and collaborative initiativesAdhere to Mercy Haven and DOH policies, procedures and regulationsProvide on-call coverage as required to support program operations and respond to client needs or emergenciesAll other duties as assignedPOSITION REQUIREMENTSMaster's degree in social work or a related field plus 1 year of full time relevant experience ORAssociate or Bachelor of Science in Nursing (ADN/BSN) with RN license plus 1 year of full-time relevant experience ORBachelor's degree in any field with significant coursework or experience in human, social, or health services plus 3 years full-time relevant experience, including 1 year in program operations or management. This 10-bed program will provide short-term, recuperative care for individuals experiencing homelessness or unstable housing who are recovering from or preparing for surgery.
