The Case Manager will work collaboratively with children, families, health care providers, educators, therapists, community organizations, and other service professionals to identify needs, coordinate resources, develop individualized service plans, and monitor progress toward established goals. This position plays a vital role in supporting Children and Youth with Special Health Care Needs (CYSHCN) and their families by coordinating services that promote optimal health, development, independence, and overall well-being.
