4 Qualifying fields include education degrees featuring a major or concentration in: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field. PURPOSE OF THE POSITION : The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program).
