Using a patient-centered approach, Health Home Care Coordinators assist individuals enrolled in the program manage their conditions to achieve optimal health outcomes by developing individualized care plans designed to link individuals to necessary services; such as, housing, health care services, mental health and substance use providers, social services (food, entitlement benefits, transportation), community programs, and more. Whitney Young Health (WYH), in partnership with Capital Region Health Connections, is committed to providing comprehensive community-based case management services to adults with chronic medical conditions and co-occurring mental health and substance abuse disorders, through Health Home Care Coordination across Albany and Rensselaer counties.
