Communicate, coordinate, and document all aspects of patient/client management including the results of the initial examination and evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in patient/client status relative to the interventions, reexamination, and discharge/discontinuation of intervention and other patient/client management activities with members of the care team as indicated. As a Physical Therapist or Physical Therapy Assistant, you will: Perform an initial examination and evaluation to establish an impairment/diagnosis, prognosis, and plan of care prior to intervention for all persons served with basic and some complex rehabilitation needs in acute care, outpatient, or inpatient rehabilitation settings (flex to any Rehab site as needed).
