li>Renders mentoring services and supports both in the office and in community settings necessary to deliver addiction counseling services and supports; works within a team; collaborates with coworkers and community partners; assists persons served in accessing the resources they need to recover from substance abuse problems; provides case management and care coordination services; assists in making timely referrals to other professionals, agencies, community programs, or appropriate resources; evaluates outcome of referrals. The incumbent in this position upholds confidentiality rules/regulations including HIPAA and 42 CFR Part 2. Services may be delivered in person or via telehealth; services may be delivered in the following locations: office or in the community including settings necessary to deliver addiction counseling services (e.g., PCC, schools, hospitals, jail, courtroom, residences including transitional housing, etc.).
