Serve as an embedded clinical partner to primary care providers, advanced practice providers, and interdisciplinary care teams to co-manage medication therapy, support clinical decision-making, and optimize individual participant medication regimens, promoting the safe, effective, and financially responsible use of medications. Actively participate in interdisciplinary team (IDT), morning meetings, and other participant-centered clinic meetings to provide medication-related expertise, conduct comprehensive medication reviews, and identify opportunities to improve medication safety, appropriateness, adherence, and therapeutic outcomes.
