Delivers and coordinates clinical services for the agency, including but not limited to making client and associate assignments, coordinating client care and referrals, assuring that client needs are continually assessed, and assuring the development, implementation and updates of the individualized plan of care if applicable and per state regulations. Provides direct supervision of Brookdale at Home clinical program and nursing associates, which includes monitoring job performance, associate training, and participating in associate performance evaluations, coaching and counseling, and conducting and documenting corrective action as needed.
